Membership Overview

The clinic offers optional membership plans designed to enhance access to care, communication with providers, and wellness services. Membership benefits are non-transferable and apply only to the enrolled patient.
Membership fees cover access to enhanced services and wellness benefits. These fees do not replace medical insurance coverage and do not apply to insurance copays, deductibles, coinsurance, or other patient financial responsibilities. Patients using insurance will still be responsible for any insurance-determined charges for medical services.

Membership Billing

Membership fees are billed monthly.
The clinic securely stores payment information using Square payment processing.
Billing policies include:
Membership payments are automatically charged on the 5th of each month
The first month may be prorated depending on enrollment date
Membership continues month-to-month until cancelled
Patients authorize the clinic to automatically charge the payment method on file for the membership fee.

Cash Pay Processing Option

Patients who choose to pay by cash instead of automatic credit/debit card billing will be charged a $10.00 monthly cash processing fee.
This fee covers administrative processing and manual payment management.
Cash payments must be received before the 5th of each month to maintain active membership status.

Grandfathered Diamond Benefit

Patients who were active members at the clinic’s previous location may qualify for a Grandfathered Diamond Membership benefit for one (1) year. Eligibility will be verified by clinic staff.

Late Payment Policy

To maintain membership benefits, payments must be received on time.
A $15.00 late fee will be added if:
An automatic card payment is declined, or
A cash-pay patient fails to make payment by the due date. If payment is not received within 10 days of the due date, membership benefits may be temporarily suspended until the balance is paid. Membership benefits will be permanently terminated if payment is not received within 45 days of the due date.

Membership Cancellation

Membership may be cancelled at any time with 30 days written notice.
Notice may be provided by:
Email Written request submitted to clinic staff.
Membership benefits remain active during the 30-day notice period, and billing will continue during this time.
No refunds are issued for partially used membership months.

Non-Insurance Services

Membership services are not insurance products and are not submitted to insurance for reimbursement.
Membership provides enhanced access and wellness services, not payment for medical treatment.
Patients using insurance will still be responsible for:
Copays
Deductibles
Coinsurance
Any services determined by insurance to be patient responsibility.

Non-Member Patients

Patients who choose not to enroll in membership may still access:
Boutique therapies
Supplements and wellness products
Laboratory services
These services will be provided on a cash-pay basis, and membership benefits will not apply. The patient will also have access to classes, events and nursing services but will not qualify to schedule one on one visits with any Lou Life providers.
